| Notes | 1 The May 8, 2013 Pharmaceuticals and Therapeutics Committee Meeting was not held because there was not a quorum. The next meeting is scheduled for Wednesday, November 6, 2013. 2 Travel Reimbursement can not be provided, in the Reimburseent section, for former P&T Committee Members who resigned or were not reappointed during the 2012-2013, 2013-2014, or 2014-2015, 2016-2017SFYs. 3 The May 6, 2015 Pharmaceuticals and Therapeutics Committee Meeting was not held because there was not a quorum. 4 The April 26, 2017 Pharmaceuticals and Therapeutics Committee Meeting was not held because there was not a quorum. 5 The Pharmaceutical and Therapeutics (P&T) Committee meeting scheduled for October/November 2017 was not scheduled to due to the lack of a quorum prior to the meeting. 6 The November 6, 2019 Pharmaceutical and Therapeutics Committee meeting was not held. A quorum was established, however enough physicians were not present. 7 The Pharmaceutical and Therapeutics Committee does not have expenditure authority. Funds for travel reimbursement and other expenditures are placed in LDHs budget. 8 LDH will not have a Spring P&T Committee Meeting due to the COVID-19 pandemic. 9 LDH did not have a Fall P&T Committee Meeting due to the ongoing COVID-19 pandemic.
